In a phase 3 study of 808 people with muscle-invasive bladder cancer at 158 sites, perioperative enfortumab vedotin plus pembrolizumab reduced the risk of progression, recurrence, or death by 47% compared with cisplatin-based chemotherapy. At two years, roughly 80% of patients in the combination group and 66% in the chemotherapy group were event-free. Serious side effects were more frequent with the new regimen, so the survival result does not erase treatment tradeoffs.
